Calling all models for the next Lester's Model Search!!!

For any person who is under 18 years of age the parent or legal guardian must complete and sign the form for legal consent. Signature is required on this form.

If applicant is chosen to model in any Lester's advertisements and promotional materials, model will be required to sign a standard photo release. Lester's will compensate models with a $100 Gift Card to use at any Lester's location.

Attach your photo only of the entrant to the supplied form. Please write your name/phone on back of photo.

Once image is presented and if you are chosen you hereby give consent to Lester's and all agents or employees of Lester's to use your image in recording, video or online. This gives Lester's the right to make any recordings of me in the form of photographs, video or audio recordings. The recordings/images can be used presently and in the future with no purpose or time limitation. I further give consent for my identity to be released in association with the recordings/images made of me. I release the right to exhibit the recordings to Lester's and all agents and employees of Lester's for private or public use including but not limited to online. I understand there will be no future compensation made to me for subsequent use of the material. I have read and agreed to the Official Rules and hereby give my consent for his/her entry into the Lester's Style Contest.
